Accident Investigation

Course Aim

This course aims to provide a good understanding of the accident investigation and root cause analysis process(RCA) through informative and involved discussion, case studies, group table-top exercises utilising and practicing the key methods and tools involved in RCA.

  • Understanding legislative implications
  • Safety management systems refresher
  • Human failure overview
  • Differentiating between systemic and human failure
  • Using timelines to gain a broader perspective
  • Establishing mutiple causation
  • Using ‘five why’s’ to find deeper causes
  • Using and collating information to understand causes
  • Generating solutions to ‘cause safety.’

Aimed At

Anyone involved in a company’s near miss or incident investigation process, ideally with a pre-existing basic understanding of health and safety who may be called upon to review incidents and undertake investigation and RCA.
