  • Course Aim:


    • Learn to take an objective view of an incident and establish genuine causes to prevent future incidents rather than apportioning blame. • Understand the different types of human error. • Develop the ability to differentiate behavioural causes and management system failures. • Familiarisation with the various RCA investigation methods and tools such as timelines, Ishikawa diagrams and the ‘5 Why’s’ process.
