GRILLON SAFETY ALERT: REQUEST FOR INSPECTION

Petzl requests the inspection of certain GRILLON products following the detection of ropes mounted incorrectly in the rope adjuster of recently produced GRILLON products. In such a case, the product does not perform its blocking function, which can result in a risk of injury. Please immediately stop sales and use of GRILLON products manufactured between October and December 2023, and follow the inspection procedure described below.

Safety Notice/Product Recall

Following on from our inspection notice issued 17 November 2023, our team has been intensively investigating the potential risk and the root cause of the problem.  Our investigation identified that the press fixture used to secure the assembly of the SafeLock button moved outside of specification, creating the potential for a faulty press fit of the button, which can lead to the button turning open and coming apart.

STOP USE
Due to the risk of unintentional opening of the device, which could result in a life-threatening situation, we are issuing this notice instructing users to stop using their device, if it falls within the range of serial numbers:

LK100A1.2: 170431 – 177209; or LK101A1.2: 170433 – 176974.

 

STOP USING ANY OF THE ABOVE ITEMS IMMEDIATELYAND REMOVE THEM FROM SERVICE

 

Manufacturing corrective actions have been introduced to repair and ensure that the press fixture remains within specification; and additional procedures have been introduced to ensure that the SafeLock buttons are secured correctly.  Validation testing has been successfully completed and production of UltraLinks has resumed, with this new process in place.

What is the issue?
3M has notified us that it has been receiving a raw material from its supplier that contains Bis(2-ethylhexyl) phthalate (DEHP) above the allowed limit according to RoHS Directive 2011/65/EU (RoHS). This raw material has then been used in the manufacture of certain faceseals, shrouds and helmet covers.
The affected M-Series Faceshield and Helmets that use these faceseals, shrouds and helmet covers continue to meet all the respiratory protection requirements, as well as the eye, face and head protection required by the standards.
The products have been evaluated by toxicology professionals in the 3M Corporate Toxicology and Environmental Science Department. Based on these reviews, these products are not expected to pose a health risk when used as intended and in accordance with the instructions for use.

What do you need to do?
If this notice regarding an instance of non-compliance is of concern and you require a replacement part, you will need to remove the faceseal from your existing head top and return it back to us. We will then issue you a replacement faceseal free of charge which you will then need to fit back to your head top before continuing to use as normal.

For those working in high heat environments where sparks, hot particles or molten metal are present, there is currently no replacement faceseal available. If you have any concerns or wish to discuss replacement options, please contact your Account Manager in the first instance.

M1 SCBA Catch spring/Retainer PR18 Pressure Reducer

April, 2023

PLEASE READ NOW AND TAKE ACTION

 

Dear MSA Customer,

MSA is issuing this User Safety Notice to inform you of an inspection procedure to verify the proper operation and function of the U-clip and catch spring used to retain the PR18 Pressure Reducer of the MSA M1 SCBA (figure 1).

 

MSA has received a report from a customer that recently found isolated instances of a missing retention screw and a deformed catch spring. The PR18 pressure reducer is secured to the backplate by a U-clip, which is held in place by the catch spring when the catch spring is properly attached to the backplate via the retention screw. If the retention screw is missing, or catch spring is deformed, there is potential for the U-clip to disengage, allowing the reducer to move, which can lead to disconnection of the medium pressure line, and exposure of the user to the surrounding environment. MSA has not received any reports of injuries associated with this condition. However, it is necessary that you stop use and perform the actions outlined in this notice.

If you are unsure if your unit is an M1, reference the product label found on the backplate of the unit (figure 2)

Inspection Procedure

 

The inspection must be performed immediately on your entire stock and is to ensure the prescence of the retention screw and catch spring, as well as the operation and function of the catch spring with the U-clip.

 

  1. Visually inspect to confirm the retention screw and catch spring are assembled. (figure 3)
    1. If these are not present, remove the unit from service, isolate in quarantine, and contact MSA.
  2. Once assembly is confirmed, pull the U-clip towards the left side as shown. (figure 4)
    1. If the U-clip is not secured, push it in fully and repeat pull test.
    2. If the U-clip does not remain secured, remove the unit from service, isolate in quarantine, and contact MSA.

MSA has also updated the M1 SCBA service manual to include an additional inspection procedure for the U-clip and catch spring, available here: (D2018711 – M1 SCBA Service Manual)

Please have your trained service technician download and replace your manuals with the latest update. These inspections must be performed during each disassembly or assembly of the SCBA.

User Safety Notice

To all MSA Fall Protection Customers

MSA is issuing this User Safety Notice to inform you that MSA has received field reports of a limited number of Latchways Standard Self-Retracting Lifelines (SRL) in which some internal bolts came loose. MSA has not received any reports of injuries associated with this condition. However, we are requesting that you perform the actions outlined in this notice.
Over time, the loose bolts will be identifiable by the user as extraction of the cable from the SRL housing will no longer be possible. However, MSA’s investigation has determined that this may be preceded by a window of time in which a fall may not be arrested.
